Get Healthy Information and Coaching Service
The Get Healthy Information and Coaching Service is a free telephone and web based service staffed by qualified health coaches aimed at supporting NSW adults at risk of developing chronic disease to make lifestyle changes in the following areas:
- Physical activity
- Healthy eating
- How to reach and maintain a healthy weight
The Service will take calls Monday to Friday from 8am to 8pm starting on 23 February 2009. People will be able to access the Service by calling 1300 806 258. 2009. People can refer themselves to the service or be referred by their General Practitioner or Practice Nurse. For patients who are referred by the Practice team to the service, there will be ongoing communication and regular reports to the GP on the patients progress.
GP NSW has developed a Medical Director and Best Practice referral template for the service. Instructions are also available for importing templates into Medical Director and importing templates into Best Practice.
All people who call the service will be asked a number of questions about their health and well being. If these questions highlight any relevant health issue, the participant is referred to their General Practice to obtain a medical clearance.
Attached are two mock up reports for the GPs. They receive a report at 12 and 26 weeks and they are faxed to the General Practice.

Q: Does the feedback only go to the GP if the GP has made the referral - or does it also go to those GPs where there has been medical consent required for the patient to participate as well? |

A: GPs will receive a progress report at 12/26 weeks when they refer to the Service, or if they provide the medical clearance form to the Service. |

Q: Are the letters posted to the GP in hard copy, or faxed or emailed or a combination? |

A: The Letters are sent by hard copy in the post to the GP. The Service has not got a comprehensive email list for GPs, but this is something we could look at in the future if you think it would be beneficial. |

Q: Does the patient get a copy as well to take to their GP when they next go? |
|
A: The GP only receives the GP letter, the patient doesn't receive it. |
