GP NSW Support to NSW Divisions
GP NSW has been contracted by AGPN to support Divisions in implementation of this initiative. Support to Divisions will include:
- Support to NSW Divisions to send one representative to be trained as a facilitator for RESET for Life program
- Information and support to Divisions in using Minimum data set for capture of patient information
- Telephone help desk and information on implementation as required by Divisions
- Distribution of support tools and materials to Divisions as they become available
Regular updates regarding the Prevention of Type 2 Diabetes Program are included the fortnightly 10 minute update.
