Aboriginal Employment Strategy

Recruiting and retaining Aboriginal staff as part of the National Partnership Agreement on Closing the Gap in Indigenous Health Outcomes is a new challenge for many Divisions. GP NSW has worked in partnership with The Echidna Group - Indigenous Research & Development Consultancy to develop and implement an Aboriginal Employment Strategy within participating Divisions in order to provide sustainable employment opportunities for Aboriginal people.

The Echidna Group has developed an extensive guide for Divisions on how to successfully recruit and retain Aboriginal staff.

Care Coordination & Supplementary Services Resources

If you are a new Care Coordinator you need to be able to use our patient data online reporting tool (MMEX). GP NSW has developed a CCSS online reporting platform (MMEX) for Care Coordinators to enter de-identified patient data. The online reporting tool is located at: https://mmex.gsmhn.com.au. You will need a username and password for MMEX. If you do not have an MMEX login Jerry Bacich will organise this for you. MMEX video tutorials, cheat sheets and other training resources on how to use the online reporting tool for CCSS are available at http://staging.gpnsw.com.au/ccss/.

Department of Health and Ageing - Care Coordination and Supplementary Services (CCSS) Program Guidelines can be downloaded from http://www.health.gov.au/internet/ctg/publishing.nsf/Content/improving-frontline-health/$file/ICD-Package-Care-Coordination-and-Supplementary-Services-Program-Guidelines.pdf. This document describes the Indigenous Chronic Disease Package CCSS Program and provides guidance for organisations and individuals in the implementation and management of the Program. Please ensure you are familiar with these guidelines as they provide you with intended scope and context of your role.

Guidelines on the Use of Supplementary Services Funds - GP NSW has developed this fact sheet based on experiences from first round CCSS sites along with advice from the Department of Health and Ageing. The guidelines are attached for your reference. It is strongly recommended that Care Coordinators and their partners incorporate and adapt these guidelines to meet the local needs of their CCSS program.

How to claim a Medicare benefit for Specialists when using Supplementary Services funds - GP NSW has developed this fact sheet in line with advice from Medicare Australia to assist you in making appropriate claims under Medicare and to manage the admin process associated with claiming for Specialist Services.

Alternative options for services and equipment not eligible for Supplementary Services funding - GP NSW has developed this fact sheet to provide advice on alternatives for funding equipment, assistive devices and other required supports which fall outside the allowable scope of Supplementary Services funds.

Indigenous Health Chronic Disease Resource - Clinical Practice and Decision Support Guidelines developed by the Department of Health and Ageing as an Indigenous chronic disease specific web-based primary health care clearing house resource, it provides access to clinical practice guidelines, tools and patient education material relevant to prevention and management of chronic disease.

The Australian Indigenous HealthInfoNet is a 'one-stop info-shop' for people interested in improving the health and wellbeing of Indigenous Australians. It makes published, unpublished and specially-developed material about Aboriginal and Torres Strait Islander health freely accessible to people involved in the area of Indigenous health with the aim of enhancing their knowledge and skills, and improving their practice and/or policy work.

Table of NSW Chronic Disease Management Care Coordination Programs - GP NSW has developed this overview of the different care coordination programs available to eligible patients in NSW. The table lists the key program elements, patient eligibility criteria and evaluation and reporting requirements. The table includes information on the NSW Health Connecting Care Program, CCSS and the Department of Veterans Affairs Coordinated Veterans Care Program.
